Self Determination Program

What is the Self-Determination Program (SDP)?
The Self-Determination Program gives people with disabilities more choice and control over the services and supports they receive by allowing them to manage an individual budget.

How is SDP different from traditional regional center services?
In SDP, individuals choose their own services, providers, and supports and manage a personal budget instead of receiving pre-selected services from the regional center.

Independent Facilitator

What is an Independent Facilitator (IF)?
An Independent Facilitator is a trained professional who helps individuals and families understand SDP, develop plans, and navigate the program.

 

What does an Independent Facilitator help with?
They can help with:

  • Understanding SDP rules

  • Creating a Person-Centered Plan

  • Preparing the Individual Budget

  • Communicating with the regional center

  • Supporting ongoing plan updates

 

Can I choose my Independent Facilitator?
Yes. Participants choose their own Independent Facilitator from approved providers.

 

Does the Independent Facilitator make decisions for me?
No. The facilitator supports and guides the process, but all decisions remain with the individual.

 

How is the Independent Facilitator paid?
Payment typically comes from the individual’s SDP budget, following approved rates and guidelines.

Person-Centered Planning

What is Person-Centered Planning?
Person-Centered Planning focuses on the individual’s strengths, goals, preferences, and needs, rather than fitting the person into existing services.

 

What kinds of goals are included in a Person-Centered Plan?
Goals may include:

  • Education or employment

  • Independent living

  • Community involvement

  • Health and wellness

  • Skill development

How often is the Person-Centered Plan updated?
Plans are reviewed at least annually or whenever the individual’s needs or goals change.

How does Person-Centered Planning connect to SDP?
The Person-Centered Plan guides how the Individual Budget is used and ensures services align with the person’s goals and values.

 

Can my goals change over time?
Yes. Person-centered planning is flexible and designed to grow with the individual.
